2022-2023 WINTER SPORTS PHYSICALS INFORMATION

Winter sports physicals will be provided to all Beaver students in grades 7-12, at NO COST, on the following date in the high school nurse's office:

Monday, November 14th (2pm-4pm)

Students must sign up and bring their completed Section 6 form from PlanetHS. Click here to sign up your student(s)

Students can still choose to complete their sports physical exams on their own. All completed physical exams must be uploaed to PlanetHS. ALL PHYSICALS (SECTION 7) MUST BE DATED ON/AFTER JUNE 1, 2022 TO BE VALID FOR THE 2022- 2023 SPORTS SEASONS. THERE ARE NO EXCEPTIONS TO THIS PIAA POLICY.

REMINDER: students AND parents should complete their PlanetHS registration first then print out PIAA CIPPE Sections 6 & 7 for their sports physical. CLICK HERE to learn how to print out PIAA Sections 6 & 7 from Student Central.

Students that participated in a fall sport DO NOT require another physical exam. They will only need to log into their PlanetHS account (student & parent) and sign the Section 8 Recertification Form.
