Student Travel Release Form

To be used when an alternative means of transportation to and from an athletic contest occurs. Must be completed by parent/guardian and returned to the athletic office for approval no less than 24hrs prior to the contest.

Beaver New Student-Athlete Information Form

All new student-athletes to the Beaver Area School District in grades 7-12 are required to complete this form and submit to the athletic office.

NEW PIAA CIPPE (Updated 2/2022)

All students must have a completed PIAA CIPPE on file prior to participating in any official practices.

All students must complete and return their emergency contact form to their head coach.

Beaver Parent/Guardian Code Of Conduct

Section 9 (Medical Clearance)

Should a student-athlete suffer an injury which requires attention from a physician, the following form must be completed and returned to the athletic trainer.
